Fiscal Year 2027 Hospital-Acquired Condition Reduction Program: Hospital-Specific Reports and Scoring Calculations Review and Correction Period Information

The Centers for Medicare & Medicaid Services (CMS) released the fiscal year (FY) 2027 Hospital-Acquired Condition (HAC) Reduction Program Hospital-Specific Reports (HSRs) on Wednesday, September 9, 2026 via the Hospital Quality Reporting (HQR) system. The HAC Reduction Program HSR contains information about a hospital’s payment reduction status, effective October 1, 2026, and on hospital performance for the Centers for Disease Control and Prevention’s National Healthcare Safety Network hospital-associated infection and the CMS Patient Safety and Adverse Events Composite measures. The report comes with a user guide with information about the contents of the HSR. CMS gives hospitals 30 days to review their HAC Reduction Program data and submit questions about the contents of the HSR. The FY 2027 Scoring Calculations Review and Correction period will begin September 10, 2026 and end October 9, 2026. 

Accessing the HSR

HSRs can be accessed directly from the HQR system (login required). Follow the steps below to access your HSR via the HQR system. You can view a brief instructional video on how to access your reports.

Step 1: Log in to the HQR system using a Health Care Quality Improvement System (HCQIS) Access Roles and Profile (HARP) account

  • The HQR system now requires users to have a HARP account with access to Unified File Management/Managed File Transfer to log in. If you have a HARP account, visit the HQR login page and log in using your HARP user ID and password. If you do not have a HARP account, you may register for a HARP ID.

Step 2: Access your HSR in HQR

  • Log in to the HQR system using your HARP ID credentials and navigate through the steps listed below to download your HSR:
    • From the left-hand navigation menu, select “Program Reporting”.
    • Then select “Measure details”.
    • Next, select the “Measure detail dashboard”.
    • On the Measure Detail Dashboard, select the program in which you are interested (HACRP). 
    • Select the program year in which you are interested (2027). If applicable, select the name of the facility you are interested in and its CMS Certification Number (CCN).
    • Your hospital’s performance results will appear. Click on the header of each section to expand the data table within each section (e.g., click on “Total HAC score performance” to expand the Total HAC Score performance data table).
    • To download your hospital’s patient- or performance-level CSV files and performance-level PDF, select “Export,” and then choose the option you are interested in. The files will be downloaded through your browser. Once downloaded, open the ZIP file to view your hospital’s information.

Scoring Calculations Review and Correction period

The Scoring Calculations Review and Correction period enables hospitals to review the results in their HSR and submit questions about their calculations. Hospitals cannot request corrections to the underlying claims data or add new claims to the data extract during this period. Hospitals can review the data in their HSR and replicate their payment reduction and component results using the information in their HSR and accompanying user guide.

CMS encourages hospitals to review their HSR and submit questions about their calculations as soon as possible. Submit all requests for corrections using the QualityNet Question and Answer Tool between September 10, 2026 and 11:59 p.m. PT on October 9, 2026.

To submit a question using the QualityNet Question and Answer Tool, select “HACRP - Hospital-Acquired Condition Reduction Program” from the program list and “HACRP Review & Correction Requests” as the topic. Hospitals should describe each discrepancy and include the following:

  • CCN
  • Hospital name
  • Measure(s) or ID number in question

Hospitals should not include personally identifiable information (PII) or protected health information (PHI) in their inquiry. When referring to the contents of the HSR, use the ID Number.
