Patient Safety Indicators (PSI)
Patient Safety Measures Overview
The Centers for Medicare & Medicaid Services (CMS) patient safety measures reflect hospital quality of care for adult patients and focus on potentially preventable complications, iatrogenic events, and deaths related to medical or surgical inpatient hospital care.
One of these patient safety measures, the Patient Safety and Adverse Events Composite (CMS PSI 90), has been endorsed by the CMS consensus-based entity (currently the Partnership for Quality Measurement), and is based on ten component measures:
- PSI 03 – Pressure Ulcer Rate (pressure injury)
- PSI 06 – Iatrogenic Pneumothorax Rate (collapsed lung that results from medical treatment)
- PSI 08 – In-Hospital Fall-Associated Fracture Rate
- PSI 09 – Postoperative Hemorrhage or Hematoma Rate (bleeding or blood clots requiring a procedure after surgery)
- PSI 10 – Postoperative Acute Kidney Injury Requiring Dialysis Rate (kidney failure requiring dialysis after surgery)
- PSI 11 – Postoperative Respiratory Failure Rate (respiratory failure after surgery)
- PSI 12 – Perioperative Pulmonary Embolism or Deep Vein Thrombosis Rate (blood clots in the lung or a large leg vein after surgery)
- PSI 13 – Postoperative Sepsis Rate (blood stream infection after surgery)
- PSI 14 – Postoperative Wound Dehiscence Rate (a wound that splits open after surgery on the abdomen or pelvis)
- PSI 15 – Abdominopelvic Accidental Puncture or Laceration Rate (accidental cuts and tears requiring a corrective procedure after abdominal or pelvic surgery)
CMS also reports the following standalone patient safety measure, which has been endorsed by the CMS consensus-based entity:
- ISCMR - Thirty-Day Risk-Standardized Death Rate among Surgical Inpatients with Complications (Inpatient Surgical Complications Mortality Rate [ISCMR])
CMS PSI 90 supports two CMS initiatives:
- Public reporting on Medicare.gov
- Hospital-Acquired Condition (HAC) Reduction Program*
CMS ISCMR supports the following CMS initiative:
- Hospital Inpatient Quality Reporting (IQR) Program
