FY 2026 Hospital Readmissions Reduction Program Mock HSR: Measure detail dashboard (08/11/25)This Portable Document Format (PDF) provides an example of the Measure detail dashboard in the HSR using mock data. The mock HSR shows how CMS presents results for hospitals that did not receive an HSR, for example, because they were not open as of April 9, 2025.

• Performance summary: This section of the Measure detail dashboard includes a hospital’s payment information as well as an overview of a hospital’s performance across the six readmission measures based on mock hospital-level data.
• Dual proportion information: This section of the Measure detail dashboard displays information about a hospital’s dual proportion based on mock hospital-level data.
• Unplanned readmission measure results: This section of the Measure detail dashboard displays performance information for the AMI, COPD, HF, pneumonia, CABG, and THA/TKA readmission measures based on mock hospital-level data.

FY 2025 Hospital Readmissions Reduction Program Mock HSR (08/09/24) This file provides an example HSR using mock data. The mock HSR shows how CMS presents results for hospitals that did not receive an HSR, for example, because they were not open as of April 10, 2024.

• Table 1 - Payment Adjustment: This table includes mock hospital-level dual proportion, peer group assignment, payment reduction percentage, and payment adjustment factor information.
• Table 2 - Hospital Results: This table includes an overview of a hospital’s performance across the six readmission measures based on mock hospital-level data.
• Tables 3-8 - Discharges: These tables include examples of patient data for each of the six readmission measures based on mock discharge-level data.

FY 2023 Hospital Readmissions Reduction Program Mock HSR This file provides an example HSR using mock data. The mock HSR shows how CMS presents results for hospitals that did not receive an HSR, for example, because they were not open as of May 11, 2022.

Note: as finalized in the FY 2022 IPPS/LTCH PPS final rule, CMS is suppressing the pneumonia readmission measure in FY 2023 HRRP payment reduction calculations due to COVID-19’s substantial impact on this measure (86 FR 45254-45256). CMS is still calculating measure results for the pneumonia readmission measure and the results are included in the HSR. However, the pneumonia readmission measure results do not contribute to FY 2023 payment reduction calculations.
